Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. The following insurances are accepted:
If you have any of the above insurance, the office will complete and process all the necessary paperwork for billing; you will only be responsible for your deductible and/or co-pay per session.
If you would like to utilize your “Out of Network” insurance benefits, the office will provide you with a “super bill” that you can submit to insurance for reimbursement within the confines of what your plan allows.
